Clinic

Students are permitted to visit the clinic with a pass from their teacher or at prescribed times if they have a medical condition that requires medication or treatment.

If your child becomes ill or hurt during the school day, we will follow the instructions on your Emergency Student Information Form if we cannot reach you. It is very important that the information contained on this form be accurate. Please report any changes in the information on this form. If needed you may download a blank copy of this form below.

Medication Policy and Procedures

Medicine cannot be administered by the school without proper authorization. Students who need a prescribed medication must have the appropriate form signed by their parent or guardian. The parent or guardian must bring the medication in an originally marked bottle to the school.

FOR SAFETY AND SECURITY REASONS, MEDICINE IS NOT TO BE SENT TO THE SCHOOL WITH THE CHILD OR SIBLINGS.

This medicine will be taken and distributed through the clinic only. Notes from home WILL NOT be accepted as authorization for dispensing medications. The authorization for medication is available below. In cases where this presents a hardship, the principal or parent may request the bus driver to supervise safe transportation to and from home via a locked box on the bus.
